To print a copy of your child’s form for school, sports, or camp from the Patient Gateway:
- Log in to your Gateway account and access your child
- Please go to the “Menu” Blue Circle on the top left
- Open to “Communication”
- Then to to “Letters” underneath
You will be able to access the form as well as any letters we have created after your Gateway enrollment.
